Please complete the Consent for Treatment of a Minor form which authorizes our providers to administer medical care or treatment to individuals under the age of 18. This form must be completed and signed by a parent or legal guardian.

Please complete the Medical Records Release Form in order to have medical records from another provider released to Midway Medical Center. You may return your form to either office location in person, by fax, or by mail.

To inquire about becoming a new patient at Midway Medical Center, please print and complete the Prospective New Patient Form. You may return your form to either office location in person, by fax, or by mail. Once your form is received, we will check physician availability for a new patient and our office staff will contact you.
