How do I sign up for the Patient Portal, and what can I do with it?

New to using our Patient Portal?
This is a secure email through our secure server. It only takes minutes to sign up for this access. You will be given a user name and password and will receive periodic updates through your personal e-mail address on file. You can access your personal health record from a smartphone or any computer with an internet connection.

Where can I find patient forms, and how do I submit them?
Please complete the Patient Registration Form and bring it with you to your visit. This will speed up processing when you arrive for your appointment.
Please complete the Consent for Treatment of a Minor form which authorizes our providers to administer medical care or treatment to individuals under the age of 18. This form must be completed and signed by a parent or legal guardian.
Please complete the Medical Records Release Form in order to have medical records from another provider released to Midway Medical Center. You may return your form to either office location in person, by fax, or by mail.
To inquire about becoming a new patient:

Once your form is received, we will check physician availability for a new patient and our office staff will contact you.

What are your office policies regarding appointments, records, prescriptions, and insurance?
Midway Medical Center strives to keep patients on time. The physicians ask that patients arrive 15- 20 minutes before their scheduled appointment time. This allows our staff to update patient information. Patients who arrive late may be rescheduled.
Patients will be required to complete a new registration form yearly along with insurance information. This form allows us to update personal information, gives us permission to treat the patient and file your insurance. This is required by all insurance companies.
Medical Records Release:
The content of your medical records will remain confidential and will be released only with your written authorization. The State of NC allows us 30 days to process your request, but Midway tries to have your records available in 7-14 business days. We use a third-party company to copy and send all of our medical records. Charges apply: the rate is set by the State of NC.
Medication Refills:
Please anticipate the need to have a prescription refilled and allow up to 48 hours for the prescription approval and processing. When a prescription needs to be refilled, please call your pharmacy.
During evenings, weekends, and holidays, routine prescriptions cannot be refilled. Please contact the office during weekdays and before any medication has completely run out.
** Narcotics and sedative medicines require a written prescription and will not be refilled after hours, holidays or weekends.
Financial Arrangements:
Our physicians are dedicated to providing you with the highest quality medical care while maintaining cost efficiency.
Insurance:
Insurance is a contract between the patient and the insurance company. Midway is not a party to that contract. All balances are the patient’s responsibility regardless of insurance coverage. Patients are responsible for any co-pays and deductibles (if not met) at check-in.
As a courtesy, our office will file your primary and one secondary insurance. If your insurance requires us to collect a copay at the time of services, we do not file your secondary insurance.
Insurance Card:
You will be asked to show the receptionist your current insurance card at each visit. This allows us to verify the information and assist you in collecting the benefits from your insurance company to which you are entitled.
Medicaid:
Midway Medical Center is currently seeing adult Medicaid patients (age 21 and over) on a case-by-case basis. If no doctor is available please call us in a few months. NC Medicaid Children will continue to be seen.
— We will continue billing Medicaid for currently enrolled Midway patients.
— If your insurance changes and you become eligible for Medicaid we will be happy to continue to see you as a private pay patient.

Please note: Insurance participation and benefits may vary. We recommend contacting your insurance provider directly to confirm coverage.
How do I contact the provider on call?
You may call the office during regular business hours. If after regular business hours, call our answering service at 817-589-4626 and they will page the provider on call. We want to handle these calls efficiently, therefore if your call is not returned within 30 minutes please call again. Also, please reserve these calls for actual emergencies as opposed to prescription refills, etc. Our providers will not prescribe narcotic medications or other controlled substances over the phone.
What is the best way to get a prescription refill?
Phone your pharmacy and ask them to send an electronic request for your medication.

What if I have a question for the physician after my visit?
Midway Medical Center offers a secure patient email system through the Patient Portal. Not signed up? Contact us to get started, it only takes a few minutes and an email address.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
