New Patients

To inquire about becoming a new patient at Midway Medical Center, please call first to check availability, then print and complete the following Prospective New Patient Form. You may return your form to either office location in person, by fax, or by mail. Once your form is received, we will check physician availability for a new patient and our office staff will contact you.
